'Connection refused' means that the remote host (or a firewall in
between) answered with a tcp reset. This happens for example when the
smtp service is unavailable.

BTW: Is this really no mistake?

May 5 10:44:48 houston postfix/smtp[5449]: ... (connect to
myserver.server.net[28.144.165.5]:25: Connection refused)

(note the different ip adresses, 3rd octet)
